  • Housing services and rental programs to decrease homelessness
  • Insurance, Medicare and Medicaid application support
  • Social Security application and benefits service (Supplemental Security Income and Social Security Disability Income)
  • Undocumented individuals seeking civilian services
  • Roundtrip transportation to our clinics for patients who need it

  • Preventative screenings (e.g., cholesterol, diabetes,
    depression, etc.)
  • Vaccinations

Should additional treatment options be necessary, our patients are given referrals and guidance to ensure a seamless transition of next steps for their care.

Counseling, Therapy and Specialty Services

We understand how important mental health is, so we provide our patients with convenient access to on-site Qualified Mental Health Professionals (QMHP) and other supportive services, including:

  • Therapy services (e.g., substance, group / individual / couples counseling, etc.) 
  • Nutrition treatment (e.g., diet and exercise) 
  • Health and Wellness Coaching 
  • Medication and Non-Medication-assisted Treatments
