We do this by acting as trusted liaisons between our healthcare partners and patients to connect them to the right resources and the most appropriate next level of care – from delivering physical, mental and emotional care to providing hands-on support with applying for and navigating financial assistance, housing or insurance.

How we partner with hospitals, long-term care providers and others:

Create tailored partnerships.

We coordinate with the appropriate members of your team – ranging from leadership to discharge specialists – to design a tailored partnership that provides you with a low-maintenance patient discharge process.

Patients are supported by Vivia’s on-call clinicians and preventive services center.

We provide on-site care while acting as the liaison between our partners and your discharged patients to avoid unnecessary admissions.

Coordinate proactive & preventive care.

Our case management team works with the most appropriate care services to help patients receive the care they need – and to avoid unnecessary admissions.

Streamline hospital communications.

Should patients require a hospital visit, our case management team works with your intake department to obtain immediate services and direct admission.

Direct nursing home admissions, as needed.

Patients interested in direct nursing home admissions will be assessed and referred accordingly, reducing observation hospital unit volumes.

Provide comprehensive, safe discharge planning.

Our care coordinators work with our partners to create a safe discharge plan for your patients, supported by a comprehensive care plan.
